Our Roadmap To Success
We believe that it takes knowledge, empowerment and discipline to create sustainable success!
Step 1: Qualification
- Preliminary vetting through multiple phone calls and hurdles to assess commitment level
- Invitation to Day Center for interview and explain program criteria
- Personal assessment for all family members to identify physical and mental needs
- If accepted, enter the program and placed on the road to sustainable independence
Step 2: Implementation
- Guest are provided temporary shelter, meals, transportation (as needed) and scheduled hospitality interaction with our volunteers.
- Meet weekly with Case Manager to ensure guest is following the program rules and getting proper support including social services.
- Weekly assessment to verify that guest is on plan toward the goal of sustainable independence.
Step 3: Graduation
- Guest will graduate from program within 90 days and move into their NEW home after criteria is met such as acceptable job and savings.
- Case management will monitor the graduates and offer follow on support for 12 months.
