Case Manager (CM) Job Objective

This organized, compassionate, and motivated person will quickly transition families from homelessness to permanent housing, following a rapid-rehousing, housing-first approach.  The CM will also assist families in obtaining outcomes such as securing employment, increasing earning potential, securing/maintaining housing, and/ or securing a trade/certificate.

 

This Case Manager will be responsible for our shelter program, however, will be cross-trained on all our programs and may assist with them as needed.

 

Responsibilities:

  • Lead the intake process for all potential families: conduct intake interviews including talking with referral source(s)/partners; conduct orientation for new families moving into the shelter program.
  • Communicate with the Case Management team to ensure our volunteers have necessary family information (work schedule, food allergies, etc.).
  • Assist with job applications and interview preparation; budgeting/financial coaching; getting families connected to necessary resources (daycare, counseling, educational programs, etc.);
  • Find housing for families by liaising with landlords and getting families ready as needed: debt repair, ensuring families can get utilities turned on in their name, will have furniture, etc.
  • Work with other Case Managers to deliver resources, referrals and workshops as needed related to employment, financial literacy and budgeting, housing plans, and educational planning.
  • Develop relationships with partner organizations and maintain database of resource for families.
  • Serve as an advocate for families with community service agencies.
  • Provide crisis support to families and volunteers as needed.
  • Conduct weekly individual meetings with network families (minimum) and monthly meetings with graduates. Conduct home visits and communicate in a way that is mutually convenient.
  • Caseload includes 10 families in network and est. 40-50 graduate families at any given time.
  • Additional responsibilities as needed. Family Promise provides community-based rapid rehousing and shelter diversion programs.

 

Administrative Responsibilities:

  • Work with /supervise volunteer family mentors and interns as needed.
  • Document all case management activities.
  • Track services provided (furniture, bus passes, financial assistance, gas cards, etc.).
  • On call, provide coverage for nights/weekends (approximately 1week/month).
  • Responsible for collecting and managing data, accurately and efficiently.
  • Keep case file records for every family, meeting program, funder and family needs.

 

Additional Responsibilities:                                                                      

  • Schedule is expected to include one evening shift/week and one weekend day at least once/month (or a similar combination).  Job requires flexible scheduling and will involve weekend/ nights to meet with families.
  • Must have valid driver’s license and willingness to use personal vehicle to transport guests if necessary (reimbursed for mileage).
  • Some lifting of materials/donations and help moving families into their own apartments.
  • Assistance at fundraisers and special events as needed.

 

Education, Experience and Skills  

  • The successful candidate is enthusiastic and can display good judgment, take initiative, and help families achieve outcomes. Must have a can-do, figure-it-out attitude.
  • Four-year degree (BSW, psychology, or related degree) desired.
  • Minimum 3 years’ experience in direct service related to employment placement, housing/shelter, and/or family services preferred.
  • Expected case management skills include trauma-informed care, rapid-rehousing, housing-first and a strength-based approach.
  • Must be detail oriented, execute projects in a fast-paced, often pressured environment; able to prioritize, self-manage, work independently and use good judgment at all times.
  • Must possess a positive attitude, modeling this for families we serve.
  • Must be proficient in computer programs and demonstrated record of tracking outcomes, recording case notes.
  • Comfortable working with various constituents: guests we serve, faith community volunteers, corporate and foundation partners, etc.

 

Salary and Benefits

  • This is a full-time position with benefits.
  • Starting salary is mid to high $30Ks.
  • Mileage reimbursement for work-related car travel.

 

Work Environment & Managing Success:

  • The Case Manager reports to the Executive Director
  • Occasional lifting of materials up to 25 pounds is necessary.
  • Family Promise is comprised of a small staff and a large pool of volunteers.
  • This position’s success is measured by the outcomes of the families: rate to permanent housing (80%), retaining housing (80%), length of time in shelter (less than 45 days), and maintaining a caseload of about 40 families.

Shelter Diversion Case Manager Job Objective

This organized, compassionate, and motivated person will assist families experiencing homelessness to avoid entering shelter and to secure housing.  Currently, shelter diversion support families experiencing homelessness and seeking shelter to (a). restore housing or (b) identify a safe place to avoid entering shelter. At its core, the practice is trauma-informed, client centered, and sees homelessness as a crisis that needs to be mediated.

 

Responsibilities:

  • Receive referrals from Centralized Intake and promptly follow up with families. Families who qualify for shelter diversion are at the “front-door” of shelter- they are imminently at-risk of becoming homeless and/or entering shelter.
  • Working from an asset-based, trauma-informed, housing-first approach; assist families to secure housing and work through any barriers as needed.
    • The goal is to avoid going into shelter. This may mean a family goes directly into housing or a family stays somewhere temporarily while they work toward housing.
    • In conjunction, we assist with job applications and interview preparation.
    • Work with other Case Managers to deliver resources, referrals and workshops as needed related to employment, financial literacy and budgeting, housing plans, and educational planning. Utilize organization’s resources and partnerships.
  • Find housing for families by liaising with landlords and getting families ready as needed.

  • You may be part of organization on-call system as we do provide shelter, provide coverage for nights/weekends (approximately 1week/month).
  • Keep case file records for every family, meeting program, funder and family needs.
  • Work with /supervise volunteer family mentors and interns as needed.

 

Additional Responsibilities:     

  • Job requires some flexibility and may involve a weekend/night to meet with families.

 

  • Must have valid driver’s license and willingness to use personal vehicle to transport guests if necessary (reimbursed for mileage).
  • Assist with ensuring cleanliness and safety of the Day Center, meeting the guest needs.
  • May assist at fundraisers and special events

Volunteer Coordinator (VC) Job Objective:

To ensure the volunteers and external partners provide effective hospitality to our families and that volunteers have a positive experience while engaging in Family Promise.

 

This organized, compassionate, and motivated person will quickly help with the transition of families from homelessness to self-sufficiency through resources, donations, and volunteer services for our organization. The Volunteer Coordinator will also be cross trained in other areas of the organization and may assist with them as needed.

 

Responsibilities:

  • Communicate with the Case Management team to ensure our volunteers have necessary family information (food allergies, clothing size).
  • Oversee all of Family Promise North Central Palm Beach County’s community service projects
  • Develop and manage Family Promise Volunteer Program
  • Maintain working relationships with our volunteer congregations
  • Host and facilitate our monthly Coordinators meeting (in person and via zoom)
  • Recruit, manage, and retain volunteers by allocating responsibilities and tasks (i.e., backpack drive, turkey giveaway, holiday party)
  • Ability to communicate the mission of the Family Promise North Central Palm Beach County Program and its need for volunteer assistance in the community
  • Responsible for maintaining our internal and external storage units to ensure that they are properly organized
  • Disseminate information to both internal and external stakeholders on upcoming needs and events
  • Coordinate our weekly grocery drop-off and donations at the Day Center
  • Schedule our grocery drops-off calendar for each quarter
  • Schedule our family events and group sessions in collaboration with the case management team
  • Keep detailed records of volunteer information and assignments as well donations of food and clothing
  • Engage organizations seeking volunteer/ community service opportunities
  • Develops volunteer teams for holiday/special events, as necessary
  • Must be able to work alternative schedules such as nights, weekends, and holidays

  • One-year supervision in a volunteer or operations program.
  • Experience in volunteering locally
  • Management knowledge to include:
  • Organizational skills and working knowledge of databases and MS Office to include excel
  • Ability to communicate the mission of Family Promise North Central Palm Beach County Homeless Shelter and need for volunteers
  • Experience recruiting through various channels

  • Starting salary is $15-17 per hour depending on experience
